What is Sleeve-Gastrectomy ?

What’s A Gastric Sleeve?
The rising problem of obesity causes major health problems, reduced quality of life and reduced life expectancy. A gastric sleeve bariatric surgical procedure is indicated in any adult who has a BMI over 30, has problems with their obesity and has made substantial effort to reduce their weight by lifestyle methods. 
Bariatric surgery is performed by removing a large portion of the stomach and leaving a portion of the stomach in the form of a pouch. This pouch looks like a sleeve or tube; that's the reason why the procedure is also called "sleeve surgery", "bariatric sleeve" or "stomach sleeve". 

The gastric sleeve procedure can be performed as a stand-alone bariatric procedure or as the first step of a two-staged procedure, such as gastric bypass surgery or the duodenal switch procedure. The second stage of surgery is not always required, but is an option if additional weight loss is necessary. 

The gastric sleeve surgery can usually be performed laparoscopically. Laparoscopic surgery is a minimally invasive surgical technique that allows the surgeon to operate through several small incisions. 

Gastric sleeve operating time lasts between 1-3 hours and general anaesthesia is used for the entire procedure. Your surgeon will make Small incisions in the abdominal wall to insert small trocars. After that your surgeon will inspect the stomach by laparoscope  and divide the blood vessels to the lateral side of the stomach. Before dividing the stomach into two parts, a bougie tube will be inserted into the stomach. Bougie tube will serve as a sizer for the new stomach. 3/4 of the stomach will be removed and finally, your surgeon will reattach the rest of your stomach to form the "sleeve" or "tube".

After the surgery patients have to stay at hospital for about 2-3 days. 
The candidates of Gastric Sleeve Surgery are;  

  • Both men and women who is age is between 18 and 65,
  • The ones who is body mass index (BMI) is bigger than 35 or more, 
  • The ones who is body mass index (BMI) is bigger than 40 or higher and 100 pounds overweight, 
  • The ones who have obesity-related major comorbidities such as type II diabetes (T2DM), hypertension, sleep apnea and other respiratory disorders, non-alcoholic fatty liver disease, osteoarthritis, lipid abnormalities, gastrointestinal disorders, or heart disease.
  • The ones who have made substantial effort to reduce weight by lifestyle methods but unable to achieve their goals. 
All patients are encouraged to walk as early as 3-4 hours after surgery. In general, the pain is easily manageable after surgery. Discharge occurs after 2-3 nights hospital stay. All patients must take multivitamin and B complex after surgery.

Returning to normal activities such as going back to work and/or school usually takes 2 to 4 weeks. 

First 2 weeks patients have to go on a special diet called liquid diet. First day after the surgery is a clear liquid diet, starting from the 2nd day to the 14th day a full liquid diet including protein shakes, yogurt, broth, milk, and juice are required. During the first 2 weeks because of the low calorie intake from liquid diet fatigue is common however feeling hunger is not. 

Weeks between 3-5 turning to a soft food diet, the energy level improves quickly. 

After 5 weeks patients can begin to eat regular food.  It is important to eat a small amount of food each time and have meals that are more frequent.

Time for full recovery starts by 3 weeks after surgery, patients will be able to begin exercising 5 weeks after surgery.

Can I Get Pregnant After Gastric Sleeve?

Yes you can get pregnant after gastric sleeve surgery, but you should wait at least 18 months for a safe and healthy pregnancy. It is important to stabilize weight before getting pregnant. After the surgery, your body will go through a lot of demanding changes. 

  • Gastric sleeve surgery restricts the amount of food the stomach can hold,

As your body adapts, you may experience side effects such as; feeling cold and tired, dry skin, mood changes, muscle aches and fatigue, hair thinning and loss.
Sleeve gastrectomy is a permanent procedure and cannot be undone. As they can be reversed you may consider to think about the other surgical alternatives like the adjustable gastric band and gastric bypass. All 3 surgical procedures can provide similar weight loss benefits. 

Roux-en-Y Gastric Bypass (RYGB): In gastric bypass procedure a portion of the stomach is stapled off in order to form a gastric pouch. After this step to bypass the lower stomach and intestines, the small intestine is attached to the pouch.  
Adjustable Gastric Band: In gastric band procedure a band is placed around the upper part of the stomach. This band separates the main part of the stomach from the smaller part. Gastric band surgery provides limited food intake, by filling up the smaller part of the stomach with food faster and empty slowly into the rest of the stomach. 

Gastric Balloon: Gastric balloon is a nonsurgical procedure that provides weight-loss for people with a BMI of between 27 and 40.  The aim of this procedure is to make the patient feel fuller and eat less by taking up space in the stomach. Before deflated or removed the balloon will stay in, up to six months to a year. 

Gastric Botox: Gastric botox is an non surgical procedure that helps accelerate weight loss. Gastric botox, also known as 
"stomach botox", performed endoscopically to the stomach wall. Botulinum toxin "botox" slows down the stomach so patients start to feel fuller. Stomach botox is performed under anesthesia, patients may leave the hospital two hours after the procedure.
