First of all, I would like to thank you for taking your time for answering these questions Dr. Valerio Finocchi.

What is Preservation Rhinoplasty and What Are Differences Between Traditional Rhinoplasty and Let Down Technique?

Hello everybody, It is a pleasure to receive your invitation as Turkish people they have left a mark in my life both in friendships and in professional life.

Actually Preservation Rhinoplasty is a new philosophy that includes three main components: 

1. Respect for soft tissues through a minimally invasive dissection, Dr. Baris Cakir from Istanbul was the first to propose this approach that he has taught all over the world.

2. Dorsal preservation which allow to change height and profile of the nose without ruining the original anatomy. This allow to obtain very beautiful results with a very low percentage of complications.

Actually dorsal preservation philosophy was forgotten but in reality was proposed more than 100 years ago.

Dr Yves Saban from Nice (France) was the link between old generation and new generation and it is thank to him if now most of surgeons around the world is using this wonderful technique. 

3. Alar cartilage preservation also is the last concept which we are now applying  to avoid complication at the level of the tip of the nose.

Aaron Kosins from New Port Beach (USA) showed how is possible to avoid post operative retractions and problems at the level of the tip.

The preservation rhinoplasty gained popularity for the dorsal preservation because totally different compared to the traditional philosophies.

So now we have three main philosophies for the hump treatment: 

The traditional Joseph almost abandoned because the hump was removed but the anatomy was not reconstructed. This was giving the classic operated result and people notice that the patient had been operated.

The evolution of this technique was the Structure philosophy which reconstruct the anatomy through the use of cartilages grafts or flaps in order to avoid the “operated look”.

And for last the Preservation approach which do not remove the hump but works under it at the level of the septum and nasal bones in order to reduce the height of the profile and its shape. 

The simpler example to understand the difference between structure and preservation is to think about two twin buildings and think that their height is the dorsal height of the nose.

If I want to reduce it I can remove the top floors but then I have to reconstruct the roof ( Stractural philosophy) or I can reduce it working at the level of the first floors (Preservation Philosophy).

The good thing about preservation is that we can even change the shape of the profile because the connection between bones and cartilage is a joint that moves as we want. 

Both philosophies works very well and the surgeon should master both of them and chose the right technique for the right patient.

There is not the perfect techniques for everybody but there is the best technique for the case and it is the surgeon that should choose it. Moreover now is born a new philosophy that is applying both the philosophy together to maximize the result.

Could You Please Tell Us Question Checklist for Rhinoplasty Consultation. Which Questions Should Be Asked To The Surgeon?

The consultation is very important and the surgeon should explain some important issues after he photographed and visited the patient:

1. To talk the same language is important to explain the anatomy of the nose and then is possible to explain the technique that the  surgeon want to propose. Even better if this is done showing the picture of the patient to explain what is in the surgeon project. 

2. In the second part of the consultation  the surgeon should explain how to prepare yourself before the surgery (analysis, drugs to buy) , what to do the day of the surgery (no eating before surgery, how and when to present their self at the hospital) and what to expect in the postoperative period(how to behave after surgery).

I personally give a little book to every patient because I don’t want them to forget anything I said. It is a sort of little guide that they can use when they are in doubt for something.

It is very important that the patient know the style of the surgeon and that is possible to see pre and post operative results.

Valerio FinocchiI really like the Russian nose which is small with a low radix and beautiful tip. The armony between the nose and face is great. 

I had the honor to operate few Turkish patient and I think that it is an optimal structure to work with because the average of patients have good cartilage quality and amazing skin. Great genetic and amazing food (just joking!). 

I love Turkish people, I always say that for me is a second family, I love the culture, I have great friends there that I consider like brothers.

The traditions that are transmitted troughs the centuries mix Eastern and Western cultures. I have always felt welcome and the acceptance that has been reserved for me is unparalleled.

About The Turkish woman, I was always struck by their intelligence with that seductive look. The Turkish man, a real hard worker and fighter, when it comes to rights he is always the first in line.
